Can I put my spouse/child/parent/friend on the list?

No. This is a self-exclusion program and only the problem gambler can elect to place his or her name on the self-exclusion list.

 

 

How do I remove my name from the list?

Your name is removed only when the self-exclusion duration ends. There is no process to alter or rescind the self-exclusion. It is irrevocable and remains in effect for the entire time period elected. Your name will remain on the self-exclusion list until you complete the entire exclusion period, regardless of any change in personal circumstances.

 

 

What facilities will I be excluded from?

The state-wide self-exclusion includes all Class III Indian Gaming Facilities in Arizona and includes the use or services of any Indian Gaming Facility such as the restaurants, concerts, hotels and conferences on the casino property.

 

 

Will I be excluded only from Arizona casinos?

Harrah’s/Caesars Entertainment has a policy that any person excluded from one of their casinos is excluded from all of their casinos, worldwide. Since Ak-Chin is a Harrah’s property, this means you will not be able to enter any casinos owned and/or operated by Harrah's or Caesars Entertainment anywhere. Once you exclude, Harrah's will send you a letter with a listing of their current properties.

 

 

How long does it take for the self-exclusion to go into effect?

When the properly executed self-exclusion form and picture are received in this office, it is usually processed the same day. The Tribes have direct access to the database system. You are not notified when the exclusion is received in this office or when it goes into effect. It is recommended that you consider yourself banned from all Arizona casinos as soon as the form has been mailed or call the office to confirm.

 

 

What if I violate my exclusion?

Once a self-excluded patron is identified in the gaming facility they will receive a consequence at the Tribal Gaming Office’s discretion, which may include the issuance of a citation and/or arrest and prosecution by Tribal or other authorities. A self-excluded person on the premises will be considered trespassing. 

 

 

Will you notify me when the self-exclusion is over?

No expiration notices are sent to self-exclusion participants. Tribal Gaming Offices and casino personnel maintain a list of active self-exclusions. If you have questions regarding your self-exclusion expiration please contact Self-Exclusion by phone at 602-290-2555 or by email at [email protected].

 

 

Can I extend my self-exclusion for another year?

Once your self-exclusion time period has expired, you may renew your self-exclusion for another one, five years, or ten years by sending in a new notarized self-exclusion form and providing a current picture.

 

 

Can I self-exclude in person?

In person Self-Exclusions are currently only available on Fridays by appointment only until further notice. Please do not let this stop you from excluding yourself or receiving the help you may need.  The Self-Exclusion Administrator is available to answer your questions and can be reached at 602-290-2555 or by email at [email protected]. The Self-Exclusion form is available on this website and can be mailed back to our office after being notarized. Pictures can be emailed or attached to the form.
