Event Wagering & Fantasy Sports Self-Exclusion Form
There are a number of ways you can go about excluding yourself. You can download the appropriate exclusion form on this page, fill it out in its entirety, have it notarized and mail it to the Department of Gaming along with a current photo of yourself. The form (available in a fillable .pdf) can be found in three separate sections; on the Home page, above on this page, and a Links section on the Resources page. The Event Wagering & Fantasy Sports Contests (EWFS) self-exclusion form can also be found on the EWFS tab at the top of this page. Please note: The self-exclusion will not be processed without proper notarization and a current photo. The photo may be submitted electronically via email (do not send by text); however, the original, notarized self-exclusion form must be mailed or delivered to this office.
In Person self-exclusions are currently only available by appointment only. Please do not let this stop you from excluding yourself or receiving the help you may need. Questions regarding self-exclusion can be answered by calling 602-771-4263 and ask to speak to some about self-exclusions or by email at [email protected]. The self-exclusion form is available on this website and can be mailed to the office after being notarized. Pictures can be emailed or attached to the form. In addition to self-exclusion, we offer Tribal and State funded treatment from our list of treatment providers. The list of providers can be found on this site.
DPG has recently added a remote notary option to notarize the SE form. Please review the FAQ to see what technology requirements are required and acceptable forms of ID for the remote notary option.

Should you decide to pursue any of them, please submit the forms to the address listed on the form. Do not return them to the Division of Problem Gambling.
The Everi STeP program allows you to exclude yourself from using ATMs at over 1000 gambling locations.
